Appt last week at Rheumatology Options

Hi everyone, sorry I havent been around much, varous reasons, family, work and RA. Anyway, I am still on mtx at 25mg and hydroxychloroquine at 400mg with tramadol and paracetemol. I was also taking pregabalin for my nerve pain (fibromyalgia) but the consultant has changed me to cymbalta (other name gone out of my head) start them tomorrow. He has arranged a bone scan for next Monday 8th. Then a physio appt for January. Also an appt at the Sjogren's clinic on 23rd of this month, this is for a lip biopsy and lots of other tests too.

I tried to say how much pain I am getting, all he said was "well you dont look depressed" what is that supposed to mean!!? When I ask for better painkillers, he says thats what the pregabalin/cymbalta is for. Well, it doesnt work then!!! All I get is "give it time"

I am now seriously worried about how much longer I can continue to work with the kids, two little ones is hard work anyway, let alone with RA. As I am the wage earner (my husband is ill too, heart condition) what will happen if I hafe to stop? This government is making people ill worrying what will happen to them if they do stop working.

Hello Barbara

Nice to hear from you again - you were missed.

Sorry though to hear you are not in a good way at the moment. I really do think that the
Specialists do lack in their bed side manner so to speak. I have a lady Specialist who I
must say is very nice but she was running nearly an hour late and I was rushed through
my appointment and several things I wanted to mention I did not get a chance - so
fully understand how you feel.

Hope the scan and physio go well and hope they are helpful to you

I am sorry things are so tough for you at the moment. On top of all this you are worrying - hardly suprising. I know it sounds trite, but do try not to worry about things you have absolutly no control over. You can not affect them in any way. Give yourself space to think of the things you could do instead. I had a hard enough time looking after my own children, and that was 30 years ago, so I don't have any idea of what your days are like. Perhaps you could change from babies to older children as a first move?

hi Barbara,

really sorry to hear how badly you are struggling at the moment ... i can't imagine how difficult it must be with little one's to look after and the added pressure of having to as you are the Breadwinner.

hope the Scan and Physhio are of help in getting you under control.

reading about your appointment it makes me thankful that i have such a fantastic RA Department at my Hospital. do you have a good relationship with your GP as that's where i always head off too if i am in need of help between appointments or feel i need a shoulder and he always reassures me that i will get under control eventually hard to believe right now but with his support and the Forum i have to believe it.

Thank you for the replies. Hopefully the bone scan and physio (although where I'll get time for tghe appts) will show something.
The two little ones are both 2 years old, and to be honest, once they get to 3 here in London they are at nursery anway, so its difficult to get them full time any older than around two years.
I do need to see my GP, I need to contact them, the one I like to see usually has a 3-4 week waiting list. Maybe a call for that tomorrow.

sorry to here you are struggling at the moment

i agree with rose very often consultants either are running late and haven't got time to listen or just DON't listen

with regards your GP can you get an 'on the day emegency appointment' to help you with the pain side of things--you are entitled to see a someone a medical professional within 24 hours even if it's not your usual GP or at least a telephone consultation with him/her

good luck

be pushy with the receptionist they are NOT qualified to decide whether you are 'ill' or not-- i know i am one !!!!!! or should i say just retired from being one

hope you get some help quickly

Hello Barbara

Sorry to see that you are struggling at the moment. I sometimes think that when you see the consultant your appointment is rushed. It's like you sit down and its Ping your time starts now. The last time I saw mine I was very aware that he kept looking at the clock. It gets me so annoyed sometimes. If your having problems you count the days off to you see them and sometimes come out thinking what a waste of time. I'm still trying to work out about being in pain and not looking depressed, just think thats a silly comment.

I would take Carols advice and push to see a GP. You are ill, you are not wasting the GP's time, sometimes it is hard to get past the hurdle of the receptionist. I remember many years ago when I was pregnant and sitting in the waiting room for my appointment with the midwife. A young mum to be came in, she thought she was in labour, you only had to look at her to see she was in pain. She asked if she could see the midwife for advice. This is no word of a lie the receptionist turned around to her and told her she would have to wait till everyone else had been seen because she hadn't an appointment. The next mum to be when called for her appointment said that she should be seen first, receptionist still said no. The lady went in, must of told the midwife because she came straight out to see her. Would of loved to of been a fly on the wall when clinic finished.

Thank you Carol and Paula, I can have an appt with the doctor I like to see on Nov 23rd. Dont want to get an emergency appt as I need to see them re the change in meds, as my GP would only give me one month of the 1mg pred and the duloxetine, and said I have to be seen before I can have any more. Will jus have to take anyone of the doctors, so will call again on Monday and hope for an appt sooner. Our receptionists are ok really to be honest, its just a very busy surgery with 6 GPs, so there are a large number of people wanting appts, and they are usually very helpful. Will let you know what happens. I have my bone density appt on Monday, so get that over with firstSmile

Hi Barbie,

So sorry to hear that you still haven't really got any help. I understand how much pressure you must feel to go on working, your job is sooo physical and demanding. I can't really add to what others have said but am glad that you are being seen for the scan. Nice to hear from you. Let us know how you get on.

Hi Barbara - so sorry to hear you have been so unwell and down. Did you ask why they want to do the lip biopsy? If you have SS, you are probably already on the treatment - Plaquenil, MTX etc.

Lip biopsies can be suggested out of habit, but before I would let anyone at me, I would ask what will change if you are diagnosed positive. If there will be no further treatment than you are already on, I would not go through with it. Let us know how you go on X Ailsa

Ailsa, As far as I know the consultant wants to confirm (or not) the diagnosis of Sjogren's, as he believes that not all the pain can be from the RA, he has already diagnosed fibromyalgia. I have already agreed to take part in the tests as it is in conjuction with a hospital in San Francisco, and is a Sjogren's international collaborative clinical alliance. By taking part it helps with further studies into Sjogren's and I also get all tests done with only 2 appts. I do understand what you mean, I am already on the treatment, but at least I will have a definite diagnosis. Smile

Hi Barbara,

I am so sorry to hear that you've been so unwell, and understand how you must feel. I was the main income earner in my family before I was ill health retired. The situation for us is not so bad, as my husband who is self employed is still able to earn an income during these difficult financial times.

Sorry, I don't know anything about Sjogrens, but on top of the RA, the pain must be difficult to cope with. In respect of trying to continue with your work as a child minder have you approached Access to Work, I know they also help people who are self employed. You never know they might be able to come up with some suggestions to help you manage. As an experienced childminder have you thought about providing training for people wanting become childminders, or setting up an agency for childminders? Of course this is something to think about when you're feeling better.

Hi Barbara, thank you for the positive post. I had my bone scan today, the results will go to rheumatology ready for my appt in January. Thank you for the ideas, I'm not sure how to go about it, but sure I can look into it, something to consider at a later date if things do go pear shaped. Today have upped my pred, as on 4mg I am feeling too sore to cope with the kids. Have also felt very nauseus since mtx on Friday, although I dont think its fully tghe mtx as usually by Monday I am fine. Just wish I could get to a point where I know whats for the best. How are you doing afer all the problems of earlier in the year? Love Barbara x
